ACH & Credit Card Processing Documentation Requirements

• Application and registration in the Payment Portal portal
• Articles of incorporation (Not required for sole proprietors)
• DBA certificate or business license
• Most recent bank statements from Merchant’s primary bank
• Personal Guarantee- If guarantor is too weak to support the risk, a stand-alone personal guarantee or corporate guarantee from parent/affiliated entity may be required (Not required for publicly traded corporations)
• Registered non-profit organizations may provide proof of their non-profit status (ex. IRS letter) in lieu of the personal guarantee
• Voided check or bank letter, validating the DDA into which Bank will perform credits and debits under the Merchant Agreement
If Merchant is processing 25%+ MOTO or internet transactions
• Most recent three months’ prior bank statements (if processing any methods of payment)
• Most recent fiscal year-end financial statements or corporate tax returns (personal tax returns for a sole proprietor)
• Website checklist, if Merchant is processing any internet transactions
If over $3,000,000 in volume
• Two most recent fiscal year-end financial statements or corporate tax returns (personal tax returns for a sole proprietor)
• Year-to-date financial statements
